Animal Tag Registration Request

Please fill out this form to register a new dog or cat, or to request a replacement tag. Submit the completed application including payment to register for a tag. Your pet will be registered after payment is received. 

If you require more than two (2) animal registrations, or need to reassign a tag, please contact the Clerks Department for details and regulations at 519-832-2008 x100.

2024 rates are:

Lifetime registration fee: $75.00 per animal

Replacement tag for dog or cat rate: $6.53 per tag

The personal information collected on this form is collected under the authority of the Animal Control By-law and will be used to license dogs and cats and identify the Owner(s) of such animals.  Questions about this collection should be addressed to the Clerk at 600 Tomlinson Drive, Port Elgin, 519-832-2008.
